Lunch Reservation Form for Zen Vegetarian Restaurant Shigetsu
・下記入力フォームにお客様の情報を入力し、予約のお申し込みを行ってください。
・予約ご希望日の３日前までに予約をお願いいたします。
・お申し込みをいただきましたら、篩月から予約確認のご連絡をいたします。
・ご予約は、確認メールの受信をもって確定となります。 （※確認メールは自動返信ではなく、篩月の担当者より送信いたします。お送りするまでに数日お待ちいただく場合がございますので、あらかじめご了承ください。）
（ご希望日が満席の場合、ご要望に添えない場合がございますので篩月からの連絡をお待ちください。）
・お子様がいらっしゃる場合は、備考欄にご入力のうえ、お食事の要否をお知らせください。
・食物アレルギーがある場合は、ご予約時に備考欄へご記載ください。

･To make reservations at the Shigetsu Zen Vegetarian Restaurant, please fill in the form below. Required items are indicated by an asterisk*.
･Please make your reservation at least three days in advance.
･After submitting this form, you will receive a confirmation message from Shigetsu.
･Reservations should be considered final only upon receipt of the confirmation message (the message is not an automated reply but is sent by a Shigetsu representative, so it may require several days to arrive).
(Occasionally we are fully booked on the day requested, so please wait until we confirm your reservation)
・If there are children in your group, please indicate in the "Remarks" space how many there will be and whether they will be having a meal.
・Please mention any food allergies in the "Remarks" space.
- Cancellation fees
- One day prior to a confirmed reservation: 50% of the price of the lunch
Same day as a confirmed reservation: 100% of the price of the lunch
